You want to find out if there is a report on you or your practitioner organization – either for your own interest, at the request of a potential employer, licensor, or insurance provider. Requesting the Data Bank to search for a report on yourself is referred to as doing a self-query, or using the Data Bank Self-Query Service.

Note: Complete the required information on the Self-Query Input screens and then print a copy of the generated self-query. Sign the formatted copy (in ink) in the presence of a notary public and mail the notarized form to the Data Bank. You may find it helpful to print the Helpful Hints for Submitting a Self-Query to use as a reference when completing the application.
