Deactivate an Authorized Agent

Both health care organizations and their designated agents have the ability to end an authorized agent relationship. The Data Bank Administrator for either organization can terminate the relationship using the appropriate process provided below. Once an administrator deactivates the relationship, an email notification is sent to the other organization’s Data Bank Administrator requesting that the receiving organization accept the deactivation.
